BAOHT Partner Bulletin
Stay informed and connected with us! Committed to supporting patients, families, caregivers, and health partners, we provide the latest insights, resources, and community highlights.

Know an organization that should be a Partner of the BAOHT?
Are you able to recommend an organization that would be a great partner? We are now accepting Partnership Expressions of Interest for local organizations who want to participate in the Barrie and Area OHT.
By partnering with us, your organization will be part of a collaborative network dedicated to co-creating programs and services that improve the delivery of care for residents across Barrie and surrounding areas. This partnership not only promotes resource sharing but fosters innovative solutions to local health challenges.
We’re working to build a system that is people-focused, accessible and inclusive to all members of the community. If you’re ready to join us on that journey, submit a Partnership Expression of Interest Form for your organization today. All completed EOI forms will be reviewed in September 2024 by Executive Planning Committee and Collaboration Council members.

New Palliative Care Clinical Coach
Sharing on behalf of Hospice Simcoe + Seasons Centre
The BAOHT Palliative Working Group was successful in their application to Ontario Health (OH) and Ontario Palliative Care Network (OPCN) to provide a Palliative Care Clinical Coach (PCCC) for Barrie and area.
This initiative will address the needs of the dying and bereaved who are homeless or vulnerably housed, and their care providers. The PCCC will be developing relationships with key partners, executing the OPCN Health Service Delivery Framework recommendations.
“We know that homeless and vulnerably housed people continue to die at an alarming rate, at half the age of the average Canadian. People using illicit drugs and living with mental illness continue to experience stigma and discrimination, leaving them to experience suffering and unmet needs at end-of-life. This funding will allow us to increase capacity in our community to provide high quality palliative care and bereavement supports to these individuals, their families and care providers” says Kelly Hubbard, Executive Director of Hospice Simcoe – Seasons Centre. For more information about this program or to make a referral please contact Hospice Simcoe by calling 705-725-1140, fax 705-725-1710.
